Welcome to the LTFU Study Portal Activation Page. Once you have activated the portal, you will be able to gain access to valuable study information including:
- Access to complete your study surveys online with the ability to save answers and return as needed
- Access to gift codes for completed surveys
- Ability to update your contact information on your Portal Profile
- Easier communication with the LTFU Study Team
- A place to learn about upcoming studies and other resources
Thank you for activating your portal. Please complete the following steps:
- Please use Chrome, Safari, Firefox, or Edge. Do not use Internet Explorer.
- Enter your date of birth and the key code we sent you.
- Enter your email address and create a password (which must contain at least 8 characters, including 1 capital letter and 1 number).
- Check "I'm not a robot" and click "Activate."
Now you can log in to the LTFU Study Portal.
